ELIGIBILITY REQUIREMENTS:

The Upcounty Hub's Diaper Distribution Program serves children ages 0-3 years of age.

​

If a child over the age of 3 requires diapers due to a medical condition or special needs, a note from their medical provider is required.

​

We offer diaper sizes ranging from 0 to 6, but we do not carry pull-ups.

​

We do not require proof of income.

 

To be eligible, applicants must provide proof of identity and address for the parent or guardian, as well as proof of the child's age.

​

Applications can be submitted in person via a paper form or online through our Microsoft Form.

​

Once the application is completed, applicants will receive a phone call with their appointment date and time for diaper pick-up.​

​

Documentation Required for the Diaper Distribution Program:

• Proof of identity 

• Proof of address for the parent or guardian

• Proof of age for the child (medical insurance card or birth certificate)
